When a provider lists six peptides side by side, it is tempting to treat them as equals. They are not. The regulatory and evidence status behind each name varies widely, and that difference changes what a licensed pharmacy can dispense, what it will cost, and how carefully you should read the fine print.
This piece does not rank peptides by how well they work. Instead it looks at something you can act on: where each stands legally, whether a licensed pharmacy can fill a prescription for it, and how to tell a legitimate offering from a research-use-only workaround.
Two very different regulatory categories
Start by separating FDA-approved drugs from investigational compounds. That single distinction explains most of what you will see across providers.
Semaglutide and tirzepatide are approved by the FDA and sold as branded prescription products. Tesamorelin is also an approved drug with a specific labeled use. These have gone through the full approval process, which means an established manufacturer, a defined indication, and a supply chain a pharmacist can verify.
Thymosin alpha-1, MK-677, and semax sit in a different category. They are investigational or otherwise not FDA-approved as finished drug products in the United States. Some are studied in research settings, some are marketed abroad, and some circulate mainly through channels that are not legal purchase options at all. When you see these on a menu, ask exactly what is being offered and under what authority.
How each becomes legitimately available
The only purchase channel worth discussing is a licensed pharmacy filling a valid prescription from a clinician who has evaluated you. For approved drugs, that can mean a branded product or, in defined circumstances, a compounded version prepared by a 503A pharmacy or a 503B outsourcing facility.
Compounding is not a loophole. It is a regulated activity with rules about what can be made, from what ingredients, and when. Approved-drug shortages, allergies, or dosage-form needs can support compounding, but the legal basis narrows as products come back into normal supply. A good provider can explain why a compounded version is appropriate in your case.
For compounds that are not approved and not compoundable, the honest answer is that there is no legitimate prescription channel to purchase them for personal use. Sites labeling products as research-use-only are not pharmacies, and buying from them is not the same as filling a prescription.
What actually drives cost
Price differences across providers usually come down to a few concrete factors rather than quality alone.
- Whether you are getting a branded approved product or a compounded preparation, and which type of pharmacy makes it
- The consultation model, since telehealth visit fees, follow-up cadence, and lab requirements vary
- Insurance, which may apply to an approved drug for its labeled use but rarely to off-label or compounded requests
- Bundling, where membership fees or shipping can obscure the true per-fill cost
Ask for an itemized breakdown. A provider that separates the visit, the medication, and any membership charges is easier to compare than one quoting a single monthly number.
How to vet a provider
Regardless of which peptide interests you, the vetting checklist is similar. Confirm that a licensed clinician evaluates you before anything is prescribed. Confirm that the medication is dispensed by a named, licensed pharmacy you can look up. Ask which state licenses apply and whether the pharmacy is a 503A or 503B facility.
Be direct about legal status. Ask the provider to state, in writing, whether a given peptide is FDA-approved, compounded, or investigational. A trustworthy operation will not blur those lines. One that promises access to compounds with no legal dispensing pathway is telling you something important about how it operates.
